Questions to ask about rehab costs and insurance

A short checklist to help you request a clear estimate and verify benefits before committing.

Ask for two separate answers: the provider’s full price and the insurer’s confirmed contribution for the proposed care.

Get the price in writing

An advertised price may not answer every question about the program you are considering. Request an itemized estimate for the exact service and dates being discussed. Keep your own copy, along with the name of the person who explained it.

  • What is included in this estimate?
  • Are assessment, withdrawal care, accommodation, medications, and follow-up included or billed separately?
  • What might change the total?
  • When is payment due, and what are the deposit, cancellation, and refund terms?

Ask the insurer about this specific proposal

‘Private insurance accepted’ is not confirmation that your plan will pay for a particular program. Ask intake and your insurer how they will establish eligibility and any approval requirements. Do not send policy documents or medical records to Karl’s website or a public social-media account.

  • Does my plan cover this provider and this service?
  • Is approval required before treatment begins?
  • What amount, duration, or service limits apply?
  • Will the provider bill directly, or will I pay and claim reimbursement?
  • What amount would remain my responsibility?

What ART currently says

ART’s website states that it accepts private insurance and that its services are not covered by OHIP. That statement describes ART; it does not mean all addiction care in Ontario is privately paid. Confirm ART’s current terms directly with intake and your insurer. This guide does not quote a program price or promise coverage.

If private treatment is not workable

You can look beyond ART. ConnexOntario provides free information about addiction and mental health services across Ontario. Ask about services relevant to your location and circumstances, then confirm each program’s eligibility, costs, and availability directly.

Program details and access arrangements can change. Confirm them directly with the provider. These sources inform the guide; their inclusion does not imply endorsement of Karl or ART.

A question is enough.
